What Is Form I-693?

Form I-693, Report of Immigration Medical Examination and Vaccination Record, is the medical form USCIS requires from most people applying to become lawful permanent residents (green card holders) from inside the United States. It shows that you do not have certain health conditions that could affect your application and that you have received the vaccines CDC requires.

Your civil surgeon completes the form and gives it to you in a sealed envelope. Do not open it. USCIS will not accept a form whose envelope has been opened or altered.

Who Needs an Immigration Medical Exam?

You generally need an I-693 exam if you are:

  • Filing Form I-485 to adjust status to permanent resident, including through family, marriage or employment
  • A K-1 fiancé(e) or K-2 child adjusting status after marriage
  • A refugee or asylee adjusting status, in some cases
  • Any applicant whose USCIS instructions say a medical exam is required

If you’re not sure whether you need one, check your form instructions or ask your immigration attorney.

What’s Included in Your I-693 Exam

The exam follows the CDC Technical Instructions for Civil Surgeons. At Medex, it includes:

  • Medical history review and physical exam, including eyes, ears, nose, throat, heart, lungs, skin and lymph nodes
  • Mental health and substance use screening, as required by CDC
  • Tuberculosis blood test (IGRA) for applicants 2 years and older
  • Chest X-ray if your TB test is positive, done in our on-site radiology department
  • Syphilis and gonorrhea testing for the age groups CDC requires
  • Vaccination record review and any age-appropriate vaccines you’re missing, such as MMR, Tdap, varicella, hepatitis B and seasonal flu (learn about our vaccines)
  • Completed, signed and sealed Form I-693

COVID-19 vaccination has not been required for immigration medical exams since March 11, 2025.

  3. Exam day. Check in, complete your medical history, have your exam, and get your blood test and any vaccines. Most visits take about 1 hour.
  4. Results. We review your lab results when they come back, usually within several business days. If anything needs follow-up, such as a chest X-ray or a vaccine series, we’ll call you and schedule it here.
  5. Pick up your sealed I-693. Your completed form is given to you in a sealed envelope. Submit it unopened with your Form I-485.

What to Bring to Your Exam

  • Passport or another government-issued photo ID
  • Form I-693, with Part 1 filled out but not signed
  • Your vaccination records (English translations if needed)
  • Any previous TB test results or chest X-ray reports
  • A list of your current medications
  • Your USCIS receipt notice, if you have one
  • Payment for the exam

If you’ve lost your vaccination records, don’t worry. We can do blood tests to check immunity or give the vaccines you need.

Current USCIS Rules to Know Before Your Exam

  • File Form I-693 with Form I-485. Since December 2, 2024, USCIS requires the completed medical exam to be submitted at the same time as your adjustment of status application. Schedule your exam before you file.
  • How long Form I-693 is valid. A form signed by the civil surgeon on or after November 1, 2023 stays valid while the application it was filed with is pending. If that application is denied or withdrawn, you’ll need a new exam for a new application.
  • COVID-19 vaccine is no longer required. CDC removed this requirement effective March 11, 2025.

Immigration rules change often. Always check the latest instructions on uscis.gov/i-693 or with your immigration attorney.

Immigration Exams for Families and Children

Every family member applying for a green card needs a separate I-693 exam, including children. We can often schedule family members back-to-back on the same day. TB testing applies to children 2 and older, and vaccines are based on each child’s age. Please bring every child’s vaccination record.

Immigration Medical Exam FAQs

Is Medex an approved USCIS civil surgeon location?

Yes. Dr. Mohammed Islam, MD is a USCIS-designated civil surgeon listed on the official USCIS civil surgeon locator. You can verify this by searching ZIP code 11375 at uscis.gov/tools/find-a-civil-surgeon.

How long does the immigration medical exam take?

The visit usually takes about 1 hour. Your sealed Form I-693 is typically ready within several business days, once lab results are back and any required vaccines are complete.

Can I get my I-693 faster?

Yes, expedited service is available for an additional fee. Call (718) 275-8900 to ask about the next available appointment.

What vaccines do I need for a green card?

It depends on your age and vaccination history. Common required vaccines include MMR, Tdap, varicella, hepatitis B and seasonal flu during flu season. Bring your records and the civil surgeon will tell you exactly which ones you still need. COVID-19 vaccination is no longer required.

Does insurance cover the immigration medical exam?

Usually not, because it’s required for an immigration application rather than for medical treatment. Some insurance plans cover vaccines or lab tests. Call us and we’ll check your coverage.

When should I get my immigration medical exam?

Before you file Form I-485. USCIS requires Form I-693 to be submitted together with your adjustment of status application.

What happens if my TB test is positive?

You’ll need a chest X-ray, which we can do on site. If the X-ray suggests active TB, you’ll be referred to the health department for further evaluation before the civil surgeon can complete your form. A positive blood test alone usually does not mean you have active, contagious TB.

Do children need an immigration medical exam?

Yes. Each family member applying for a green card needs their own exam and Form I-693. TB testing starts at age 2, and vaccines are based on age.

Do you have a civil surgeon who speaks Spanish or Russian?

Medex DTC can accommodate Spanish, Ukrainian, Russian speaking patients.
